The MD Wellness Soulutions Programs - Consultations and Coaching (ALL sessions are with Dr. Hall)

I. MDWS Consultations:

The "inner-view" – 30 minute introductory session - A focused conversation to identify your key health concerns and goals

              Cost: $125 (credited toward your first full session) 

Next Steps: 

• Identify the main issues you'd like to address
  
• Complete and return consent forms (required to schedule your first session)
• Fill out and return of the comprehensive medical history form and time line form

• Once your forms are received, we'll schedule your first, in-depth video consultation to explore your concerns, discuss possible testing and interventions and consider referrals, if needed
• Consultation Rate: $350 per hour - billed in 15 minute increments after the first 30 minutes (The first hour is reduced by $125 if you've completed the "Inner-View session)
• Additional Benefits: Invitation to purchase supplements from My Fullscript Store with a 15% discount (An invitation link will be sent to you)

2. MDWS Coaching program:

The "inner-view" - 30 minute introductory session: A focused conversation to identify your key health concerns and goals.

          Cost: $125 (credited toward your coaching fee)

Next Steps:

•  Identify the main issues you'd like to address
• Complete and return consent forms (required to schedule your first session)
• Fill out and return of the comprehensive medical history form and time line form
• Once all forms are received, we will schedule the initial, in-depth video session lasting from 1-1/2 to 2 hours where we will
  • Review your medical history, emotional history, labs and any other tests you have submitted
  • Consider testing and/or interventions (e.g. Recommended reading, Chakra Balancing or EFT or other)
  • If tests are ordered, a written summary of the results with recommendations will be sent to you to be reviewed in detail at the next session
• 11 - weekly, one-hour sessions via video conference will be scheduled. These sessions will include personalized education, homework, accountability, monitoring (weight, blood pressure, exercise, diet, etc.), interventions, etc.
• At the 6 and 12 week mark, a 30 minute follow up session
• Access to a members only library of resources
• Invitation to purchase supplements from My Fullscript Store with a 15% discount (an invitation link will be sent)
